If you have a canon, you do not have to fill this out for their jutsu. This is only for ones you make up for them, or one you wish for them to have. In addition, this applies for all characters: Academy level jutsu like a simple shadow clone is not acceptable. Every character will know that automatically except for Rock Lee. Any basic jutsu is a given.
Genin cannot have C-ranked jutsu, but only in special cases will they be allowed. Use common sense when ranking.
You must provide at least one flaw and give a good description.
